- Booking and questionnaire
Choose a slot, pay for the consultation and complete a questionnaire about symptoms, start date of incapacity and circumstances (work, school).
- Medical assessment
The doctor analyses the information provided and may call to clarify the history if needed.
- E-sick note to ZUS
If incapacity for work is justified, the doctor issues e-ZLA. The document is sent electronically to ZUS — your employer sees it in the system.
- Information for the patient
You will learn the outcome (issuance or refusal and any recommendations) in the patient panel or from the doctor. We do not guarantee sick leave issuance.
Pricing
Consultation in the online sick leave pathway currently costs 99 PLN. The fee covers the medical service — not the document alone. The current amount is shown before payment.
Limitations — when sick leave may not be issued
Online sick leave on our platform may be issued for a maximum of 5 days — in line with platform rules. Longer periods usually require direct contact or reassessment.
Symptoms suggesting acute illness, injury requiring tests or suspected serious disease may lead to refusal of online sick leave and recommendation of in-person care or emergency services.
Retroactive sick leave or periods far in the past may be unavailable — the doctor assesses compliance with documentation and regulations.
Sick leave does not always replace school certificates — schools may require separate documents; the doctor explains options during consultation.
Frequently asked questions
- Is online sick leave valid for my employer?
- Yes — e-ZLA has the same legal force as paper ZLA. The employer receives information from ZUS electronically.
- How many days of sick leave can I get online?
- In this pathway the doctor may issue sick leave for up to 5 days. Longer periods require individual assessment and often an in-person visit.
- Must symptoms start today?
- You indicate the incapacity start date in the questionnaire. The doctor verifies consistency with the history — retroactive leave may be limited.
- Does this cover childcare leave (ZUS Z-15)?
- This pathway concerns sick leave (L4/e-ZLA) due to the patient's own incapacity. Care for a healthy child is a separate category — ask the doctor during consultation.
- Do you guarantee sick leave?
- No. Sick leave is issued only when the doctor finds medical justification. The fee covers consultation, not the document.
- When will I know the decision?
- Usually within the booked consultation window. After e-ZLA issuance the status is visible in ZUS; details also appear in the patient panel.
- Do I need a PESEL number?
- Yes — e-ZLA requires correct patient identification (PESEL or passport in the quick pathway). Details must match ZUS records.
- Can I choose a specific doctor?
- The quick online sick leave pathway assigns an available doctor in the chosen slot. Broader doctor choice is available in standard teleconsultation booking.
